Drone toppled (Hexacopter)

I tried to take off in position mode, the drone toppled to the left. Im trying to figure out what went wrong here, any help would be appreciated. I have attached the flight log link here.

Was this a first flight, or had it flown before?

This is not the first flight. I have done several flights and none of them had this issue. I tried to flash old parameters but its not working.

Was there a change before that flight?
