You would probably need to post the flight review to know for sure.
This is a longshot, but I had a similar issue where if EKF2_HGT_REF was set to rangefinder, it was not used in terrain estimates even if my rangefinder had good measurements. Terrain estimates are used to estimate distance to bottom and help determine if the distance to bottom is valid.
Solved, needed to set EKF2_RNG_CTRL = 1
