Standard vtol flight
Here pitch_sp during transition is set to 0 and have noticed the same behaviour in many onine logs
@sfuhrer any comments why it is so or It should not be this way.

During a VTOL transition to fixed-wing flight the pitch setpoint is ramped from 0° to FW_PSP_OFF, see this line.. So it looks as intended in your log, the pitch setpoint tracking is very bad though.
Maybe it’s related to the very high vibration level you have in hover mode. The vibrations are clearly visible in the motor control signals, which beside leading to bad tracking performance additionally puts a big stress on the ESC/motors that can lead to actuation failure.
